What you need

  • Board certification: American Board of Plastic Surgery (ABPS), American Board of Cosmetic Surgery (ABCS), or other qualifying board.
  • Accredited surgical facility or hospital privileges
  • Current malpractice insurance

How enrollment works

  1. Complete the application. Use the DocuSign enrollment form or request a paper application by fax at (334) 323-0588.
  2. Get your quote. Your dedicated account executive contacts you with your rates. Quotes are instant.
  3. Start registering patients. Once approved, you receive portal access. Register each patient before their surgery date.

Important: registration is mandatory

Every patient undergoing a covered procedure must be registered in the portal before surgery. This is not optional. Practices that fail to register patients receive a warning and risk cancellation.
